Patient satisfaction surveys

You are the reason we are here and we value your feedback. Once you go home, you may get a survey from the hospital regarding your stay. We hope that we were able to provide you with very good service and a positive patient experience. Please help us by completing and returning thisvsurvey so that we may continue to improve our services for you and future patients.

Cost, billing and financial assistance

In a few weeks, you will get a copy of your hospital bill. You also may get other bills from your doctor(s), including your family doctor or specialists.

Financial assistance

We are pleased to offer financial assistance to patients with limited resources and inadequate medical insurance coverage. Eligibility is determined by total family income/assets. The patient is encouraged to apply for other assistance available to pay hospital charges (Medicaid, Medicare, private insurance) before being discharged.

To request your medical records (chart)

If you need your chart sent to a doctor for an appointment, please:

  • Fill out an "Authorization to Release Information" form. You can print this copy provided online for your convenience
  • Send this form to the Medical Records Department or to your doctor's office.
  • Your doctor can mail or fax the form to Medical Records to obtain the records needed for your follow-up care.

OhioHealth's charity care policy

OhioHealth tailors its policies to address the needs of each community it serves. In Columbus, we have a unique healthcare system where all of the not-for-profit hospitals provide high quality care to everyone, regardless of their ability to pay. This system allows OhioHealth to provide one of the most compassionate charity care policies to individuals and families who cannot pay for medically necessary healthcare services they receive at our facilities.  OhioHealth's charity care policy for its Columbus hospitals includes:

  • Charity care guidelines that provide free care for individuals and families who earn less than 200 percent of the federal poverty level
  • Sliding scale fees to provide substantially discounted care for individuals and families who are between 200 and 400 percent of the Federal poverty level
  • Hardship policy for those patients who would not otherwise qualify for charity care but have unique circumstances

Patients are encouraged to apply for financial assistance as early in the process as possible, as untimely applications for financial assistance may be denied.  Patients residing in out of system service areas are encouraged to discuss financial assistance limitations to coverage for out of panel, non-emergency care with a hospital financial counselor.

In many cases, OhioHealth offers interest free loans for up to one year to assist patients.

In addition, an uninsured discount policy became effective January 1, 2005, for individuals without insurance who do not qualify for charity care.
